What a scoping review of gamification found about adherence
Gamification has become a near-default feature of health and research apps: streaks for consecutive days of logging, badges for milestones, progress bars, leaderboards. A scoping review of gamification and incentives in mobile health apps for medication adherence takes a close look at whether these features actually move the needle, and the picture that emerges is considerably more nuanced than "gamification works."
What the review actually found
The evidence for gamification improving adherence is real but inconsistent across studies, app designs, and populations. Some interventions show meaningful short-term improvements in engagement and adherence; others show engagement with the game mechanics themselves without a corresponding improvement in the underlying behaviour the study actually cares about, taking a medication, completing a data collection task, attending a scheduled activity.
That distinction matters enormously for a research context specifically. A participant engaging enthusiastically with a streak counter isn't automatically the same as a participant reliably completing the actual protocol-required task. Gamification can improve app engagement metrics while leaving the underlying adherence problem largely unaddressed, or in some cases even distract from it.
Why gamification's effects are so inconsistent
A few factors help explain why gamification doesn't produce uniform results across different implementations:
- Novelty effects fade. Engagement with a new game mechanic often peaks early and declines as the novelty wears off, which is a genuine problem for a study running over months rather than weeks.
- Extrinsic incentives can crowd out intrinsic motivation. For some participants, turning a health behaviour into a game with points and rewards can paradoxically reduce their sense of personal investment in the underlying behaviour once the game element is removed or loses appeal.
- Design quality varies enormously. A gamification feature designed around genuine behavioural science principles behaves very differently from one that's essentially decorative, a badge added because competitors have one, without any underlying theory of why it should change behaviour.
- Population differences matter. What motivates one demographic or age group can be actively unappealing to another, and a single gamification design applied uniformly across a diverse study population is unlikely to work equally well for everyone.
What this means for designing a study's digital tools
None of this argues against using gamification in a study's participant-facing app. It argues against assuming it works by default, or treating it as a substitute for addressing the actual reasons participants might struggle with adherence.
- Be specific about what behaviour the gamification is meant to change, and measure that behaviour directly, not just engagement with the game feature itself.
- Design incentive structures around genuine behavioural principles, not just because competing apps have similar features.
- Expect novelty effects and plan for them. A gamification feature that works well in the first month of a long study may need refreshing or evolving to maintain its effect.
- Consider whether gamification suits the specific study population, rather than assuming a universal design will motivate everyone equally.
- Treat gamification as one tool among several, alongside reminders, clear communication, and genuine reduction of the task burden itself, rather than the primary adherence strategy.
The broader lesson
It's tempting to treat features like streaks and badges as a solved, standard part of any modern health app, something to include because everyone does. The evidence says the actual effect on adherence, the outcome that matters for a study's data quality, depends heavily on design quality and population fit, not on the mere presence of the feature. A gamified app that looks engaging in a demo isn't the same thing as one that measurably improves the adherence a study actually needs.
